This webinar will bring together international speakers to share examples of policy and practice from Japan, Sweden, Italy and the UK that illustrate the contribution of community hospitals to integrated care and support closer to home.

Presenters
Dr Yoshihisa Honda
Primary Care Physician, CEO of Kaita Hospital, Japan and Founder of
Community Hospital Japan

Kaita community Hospital, originally a public hospital run by the town, has been transformed into an educational facility for general practitioners providing one-stop services including preventive medicine, comprehensive outpatient care, inpatient rehabilitation and social adjustment, and home visits including end-of-life care.
in 2016, Yoshihisa founded a group called Community Hospital Japan (CHJ) that holds lectures, seminars, and workshops at the Japan Primary Care Association, with other academic societies, and independently. Currently, at least 20 hospitals in Japan position themselves as community hospitals or hospitals aiming to become community hospitals. He is also actively involved in exchanges with community hospitals in Thailand and Singapore and hosts symposiums and workshops at WONCA Asia Pacific Region. In October 2024, he joined the UK Community Hospitals Association and visited the UK to learn how these hospital teams contribute to community care. He hopes to further develop community care by deepening international exchanges between community hospitals in different countries.

Dr Mante Hedman
GP at Storuman Community Hospital, Sweden
MD and Adjunct Lecturer, General Practice, Dep for Public Health and Clinical Medicine, UmeƄ University

Mante  has worked as a General Practitioner at a rural Community Hospital for many years. Noticing that Swedish rural Community Hospitals have existed for the past century without comprehensive descriptions in international scientific journals, he embarked on a doctorate to address this gap and successfully defended his thesis on the Swedish Community Hospital Model on March 1st. As both a clinician and a teacher, he is involved in the Rural Stream, a rural clinical pathway for voluntary medical students. This program allows students to exchange part of their clinical practice at the University Hospital for practice at Storuman Community Hospital.
